problem still happens but I've done a combination of the following without having to delete bookmark.nsf:

-delete cross-certificate docs in local address book.
-delete original entry for user in certlog on server.
-after the new name is accepted by the client, remove the ChangeRequest and $AdminpOldWebName fields from the person doc.

This really stinks, as I have to move over 400 users to a new heirarchy!
